Physical Activity Referral Programme

Our Movement Matters Programme has been set up to support people living with any of our conditions to become more active and help manage their health. We provide a range of classes that are tailored to the needs of the participants, allowing exercises to be done seated or standing.

We also offer condition specific classes focused on managing your health condition through movement and exercise.

All classes are run by suitably qualified exercise professionals and are delivered both in person and online.

* Condition Specific Classes Requiring Pre-Referral Triage

Level 1 – Low intensity, chair-based classes focusing on introduction to movement and exercise. These classes are perfect for those new to exercise or looking to restart their movement journey.

Level 2 – Medium intensity classes focussing on strength, balance and cardiovascular fitness. These classes can be done seated or standing, using a chair for support.

Level 3 – Classes focusing on overall strength, cardiovascular fitness and conditioning. Great for those that have progressed through our pathway or with prior class experience.

Our sessions are only ever delivered by suitably qualified health and exercise professionals. On completing the referral form one of our team will review the information and then be in touch within 7 days. Please note list of exclusion criteria below before making a referral for yourself or on behalf of someone else.

The sessions will typically last between 30 and 60 minutes and will be at a level suitable for the participant.

The instructors will offer progressions and will also provide the participants with handy hints and tips throughout of how you can keep moving at home throughout the week.

Please note referral form must be completed in advance.

  • Unstable angina
  • Recent MI (within 8 weeks of session start date)
  • Acute heart failure or chronic heart failure (NYHA 3 or 4)
  • Symptomatic aortic valve disease
  • Hypertrophic obstructive cardiomyopathy
  • Uncontrolled tachycardia or uncontrolled arrhythmia or a ventricular rate > 100bpm
  • MRC Dyspnoea Scale 4
  • Uncontrolled hypertension
  • Uncontrolled diabetes
  • Symptomatic Hypotension
  • Ortho impairment that would limit ability to take part in exercise.
  • Neurological or cognitive impairment that would limit ability to take part in physical activity.
  • Ongoing symptoms associated with Covid-19

* There may be instances where it is appropriate to take part in low intensity physical activity should the individual have had sufficient investigations and been cleared by the HCP to take part.

Before taking part in any of our sessions we kindly ask that you make a referral via our Referral Portal. This allows our triage team to make contact with individuals to discuss the most suitable class based upon current health, ability and availability.
