How Can Pharmacy Help Patients with Heart Failure who Struggle to Manage Their Medicines and Condition?

When patients with heart failure do not take their medicines as directed, they become sicker and die earlier. As is often the case, patients from deprived areas are more likely to forget or choose not to take medicines. We plan to ask patients what might help them and then use this information to help. A specially trained pharmacist-technician will meet with patients in their homes, doing what the technician, GP, Patient, HF specialist team and carer think will help get the patient back to their best possible health.
