Do you pay tax? If so your gift of £ will be worth £ at no extra cost to you.

Gifts are only eligible for gift aid if they are personal donations. Money donated from multiple individuals or from a collection is not eligible for Gift Aid. I am a UK tax payer and I would like Chest Heart & Stroke Scotland (CHSS) to re-claim tax on all donations I have made in the past four years and all donations I make from the date of this Gift Aid Declaration until I notify you otherwise (please tick if applicable). I understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that year it is my responsibility to pay any difference. CHSS will reclaim 25p of tax on every £1 that you have given. Please notify us if you: Wish to cancel this declaration. Change your name or home address. No longer pay sufficient tax on your income and/or capital gains.
