The pandemic was a catalyst for new ways of working and highlighted how health and wellbeing is not just the responsibility of the NHS. For many people, a diagnosis or return home from hospital is just the start of their challenges.
This session is an opportunity to hear about the new care pathways being established in Health Boards that include the third sector’s expertise in providing psychosocial support, making a difference to both people’s lives and to health professionals.
