Chest Heart & Stroke Scotland (CHSS) has been supporting people with long term conditions to become ‘Champions of Self Management’ for the past three years. Through training and support they are encouraged to promote self management across Scotland.

Project update

September 2015

The ‘COSMIC for Professionals’ project commenced in August 2014 with the aim to explore the role of the professional in promoting and supporting self management. Following a review of the current evidence and two surveys (collating both patient and professional views on self-management support), a course outline was developed with four key modules. The proposed modules are outlined below:

  • Core Module: ‘Introducing Self Management’ – Exploring the evidence base & self-management definitions and identifying key skills & tools.
  • Module 1: ‘Self Management Techniques and Style’ – Exploring the different tools and approaches to support self-management.
  • Module 2: ‘Making Time for Self Management’ – Getting the best out the patient and professional interactions to influence self-management.
  • Module 3: ‘Encouraging Patient Change’ – Addressing patient resistance and moving forward.

The core module was successfully piloted before the completion of the project in September 2015, and further work is needed for the development and piloting of modules 1-3. A full report of the work-to-date can be found below under ‘related documents’.
