Chest Heart & Stroke Scotland is committed to using our expertise, contacts and experience to promote and facilitate patient and carer involvement in chest heart and stroke services across Scotland.

Health Professionals Project

We are aware that one of the key limitations of Voices Scotland is our primary focus on patients and carers. Although we have worked to influence professionals (through one to one meetings, involving them in training days, attendance at events and presenting at conferences) barriers to them embracing effective involvement remain.

We are keen to utilise our expertise in this initiative to support professionals, from frontline to senior management, in local Boards, in meeting the SHC Participation Standards 2009 and ultimately promoting a mutual NHS.

The key aims of this project are:

  • To develop resources to support frontline NHS health professionals staff to embed patient and carer involvement in providing person–centred care.
  • To develop resources to support NHS staff to embed patient and carer involvement in service development and redesign.

Self management champions

'Self management is the successful outcome of the person and all appropriate individuals and services working together to support him or her to deal with the very real implications of living the rest of their life with one or more long term conditions'

('Gaun Yersel!' Long Term Conditions Alliance Scotland (LTCAS), Scottish Government, 2008)

The Voices Scotland Team has developed a 'Self Management Champions' module which is an extension of the current training offered under the Voices Scotland Programme for chest, heart and stroke patients.

Key Aims of this exciting new initiative are :

  • Raising awareness of benefits and opportunities for supported Self Management
  • Understanding the 'traditional' vs 'collaborative' approach
  • Understanding the health economics of Self Management
  • Empowering patients to keep Self Management at the top of health and social care priorities

This module will be made available on the LTCAS website (www.ltcas.org.uk) after summer 2012 for other long term conditions organisations to utilise with support from CHSS.
