Up to 100 chest heart and stroke booklets plus up to 100 factsheets at a time are available free of charge to people living in Scotland (maximum 1 order per month, please allow 28 days for delivery)

  • orders larger than 100 booklets are charged at 30p per item
  • orders larger than 100 factsheets are charged at 15p per item

If payment is required (e.g. for over 100 booklets or 100 factsheets), please print off this form and send it with a cheque made payable to Chest Heart & Stroke Scotland.
