Through a strong combination of initiatives and services, Chest, Heart & Stroke Scotland provides the resources for people to re-connect and improve the quality of their lives.
Volunteer Stroke Service
The Volunteer Stroke Service provides a variety of services in many areas of Scotland with a particular emphasis on those suffering from communication difficulties after a stroke.
Self Help Groups
The Community Support Network is a collection of independent clubs, affiliated to Chest, Heart and Stroke Scotland. There are about 100 such clubs, with a variety of activities and services on offer.
Stroke Nurses
Stroke Nurses facilitate a smooth transition from hospital back into the community, for stroke patients and help foster independence on the part of patients and their carers.
Heart Failure Nurses
Heart Failure Nurses CHSS are working in partnership with NHS Lanarkshire to develop an area wide heart failure nurse follow up service to support patients with heart failure discharged home from hospital and living back in the community.
Welfare Grants
Welfare Grant Fund is available for those people with chest heart and stroke illnesses and Welfare Benefits Advisors are available in Lanarkshire and Glasgow.
Young Stroke Support Worker
Young Stroke Support Worker (Lanarkshire) facilitates reintegration into the community for young stroke survivors and their families. The support worker helps them adjust and adapt by addressing the occupational skills, rehabilitation, support and advice needs of the individual.
