Heart Failure Nurses

Chest, Heart & Stroke Scotland (CHSS) aims to support people affected by heart failure through specialised advice and information and a range of support services in the community.

NHS Lanarkshire

CHSS is working in partnership with NHS Lanarkshire to develop an area-wide heart failure nurse follow up service to support patients with heart failure discharged home from hospital and living back in the community. Working closely with existing services, the heart failure nurses assist patients in the self management and monitoring of the condition and offer support to families and carers, aiming to enhance quality of life.
